1. Identified Risks of Participation

Participation in football activities involves inherent risks, including, but not limited to, fractures, sprains, strains, concussions, dislocations, and other injuries.

By signing this agreement, the player and their parent or guardian acknowledge and accept these risks and voluntarily choose to participate in the Club’s activities.

BFC will provide appropriately stocked first-aid kits during all games and tournaments. Relevant staff members will also be trained in basic first-aid procedures.

2. Authorization and Consent for Medical Treatment

I, the undersigned parent or legal guardian, hereby authorize BFC staff to consent to any necessary diagnostic procedures, medical treatment, emergency assistance, or hospital care that may be required in the event of injury or illness during the player’s participation in football activities.

This authorization extends to any physician, surgeon, hospital, clinic, or other qualified healthcare provider involved in providing such treatment under the general supervision or coordination of BFC’s management.

3. Release Agreement

I, the undersigned, hereby waive, release, and discharge any and all claims for damages that I may currently have, or that may arise in the future, against BFC, its management, coaches, staff, representatives, or volunteers as a result of my child’s enrollment in and participation in Club activities.

This release includes, but is not limited to, claims arising from injuries, accidents, loss, theft, or property damage, to the extent permitted by applicable law.

4. Terms and Conditions

• Prioritization of Club Activities: The player agrees that the Club’s team commitments, matches, and tournaments will take priority over other sporting events, including external tournaments and leagues. The player may not participate in any external tournament, league, or event without prior written consent from BFC.
• Player’s Responsibility: The player is solely responsible for their personal belongings. BFC, its management, coaches, and staff will not be liable for the loss, theft, or damage of personal items, including, but not limited to, watches, mobile phones, tablets, bags, clothing, or sports equipment.
• Dress Code and Conduct: The player must wear the appropriate Club uniform during training sessions, matches, tournaments, and other official activities. Failure to comply with the dress code may result in suspension from training or other disciplinary action.
• Respectful Behaviour: Abusive, threatening, insulting, or inappropriate language or behaviour toward coaches, staff members, officials, parents, or other players will not be tolerated. Such conduct may result in disciplinary action, including suspension or expulsion from the Club.
• Security and Attendance: For security and operational reasons, the Club’s gates will close at a specified time. No player will be permitted to enter after the announced closing time.
• Registration Documents: A copy of the player’s B-Form or National Identity Card, together with two recent passport-sized photographs, must be attached to the registration form.
• Registration Fees: The full applicable registration fee is mandatory and must be paid at the beginning of each season.
• Session Fees: The Club requires payment of a non-refundable half-session fee for each relevant period, such as August to November or December to March.
• Summer Fees: A minimum fee for the summer period, covering April to July, must be paid upon enrollment, regardless of whether the player regularly participates during that period.

5. Acknowledgement

By signing this agreement, I confirm that I have read, understood, and agreed to all the terms and conditions contained in this document.

I understand that failure by the player or parent or guardian to comply with the Club’s rules, policies, and regulations may result in disciplinary action, suspension, or termination of the player’s participation in BFC activities.
